One of the biggest problems across all businesses, regardless of industry, is the issue of having repeat defects and errors occurring with data, and not being able to discern them because either:
a. The measurement systems were not designed and set up properly for their intended purpose, or
b. No one is identifying patterns in the data suggestive of repetitive errors which keep recurring.
Both of these problems lead to a failure to act in the best interest of the business, as well as the best interests of the patient population. The risks to your bottom line, compliance efforts, public perception, and, worst of all, patients is palpable and cannot be ignored.
